MetLife will send Evidence of Insurability form and may also require a Statement of Health to be completed within 30 days for you or your spouse to enroll or increase coverage amount in LTD, Critical Illness or Supplemental Term Life Insurance. Election will remain in pending status until approval or denial is received.
